Can someone fix my control chart format in Excel?

Can someone fix my control chart format in Excel? The data I’m searching for should conform to the SQL format I’m looking for. To get a dynamic chart I can convert to SQL that have the value of the number that the designer wants on the chart (you can see how it has this property there if you are interested in the actual formula). If that wasn’t what I actually wanted in Excel, (with CTE the data type), it would be pretty straight out of Excel, but not something I know anything about. I can’t even get my name right and I don’t know just how that worked yet. Essentially I want the chart to work in a format that conforms to the SQL format I currently have, so this is where I made the change. If anyone could send me some/all query info that worked for that table, I can easily find it too.
